Surgery to Resolve Sugar
Evidence has been accumulated over the years that surgery for diabetes (metabolic surgery) is the most effective therapy for Diabetes. Though all bariatric surgeries are performed to resolve diabetes and other comorbidities, diabetic surgery terminology is used for procedures performed on individuals who doesn't have morbid / severe obesity (BMI 27.5 to 32.5 kg/m*).
Why Surgery for Diabetes?
Patients suffering from diabetes are often faced with metabolic risks and an increased chance of developing life threatening complications of diabetes which include damage to the kidneys (nephropathy), eyes (retinopathy), blood vessels (antipathy), and nerves (neuropathy).
Sleeve-duodeno Jejunal Bypass (SG-DJB)
DJB is a metabolic procedure, used to treat overweight/obese diabetic patients through surgical means. It is a pylorus (lower stomach valve) preserving bypass where effectively a sleeve gastrectomy is performed as the first part of the procedure (see sleeve gastrectomy for details). Following this, the second step of small intestine anastomosis similar to RYGB or OAGB is prepared but is connected to the first part of the duodenum depending on personal choice of surgeon. The sleeve part of the procedure can be tailor-made to suit the required weight loss of the patient, which makes it a safe surgery for non-morbidly obese diabetics as well as morbid patients. Sleeve-ileal Transposition (SG-IT)
Ileal Interposition is a alternative Metabolic Surgery procedure, used to treat overweight/obese diabetic patients through surgical means without causing severe nutritional deficiency. There are 2 different versions of the operation. This technique is applied by placing ileum, which is the distal part of the small intestine, either between the stomach and the proximal part of the small intestine or by placing the ileum to the proximal part of the small intestine without touching the natural continuity of the stomach. Sleeve gastrectomy procedure is standard for both versions and the size of the sleeve is decided based on the amount of weight loss desired.
